have had 4 tine fuild remove in 4 mos. All test show no caner. I have lympodema in right arm and the swelling has been reduced greatly. Could the lymphic system have a break.?

Mari, the fluid should be tested and tuberculosis (TB) needs to be ruled out. Get sputum examination and a fresh x-ray chest, and consult a chest specialist.

Have all symptoms-plus- of accoustic neuroma

My sympyoms have been progressively getting worse over the past 2 years. I have an 80% loss of hearing in my right ear with constant (24hrs) Humming in my right ear and ringing in my left ear. I have recently started experiencing seriuos headaches and dizziness. When I go to bed it takes about and hour for symtoms to calm enough to go to sleep. I wake up several times a night with symptoms. When I get up, it takes about an hour for my symptoms to calm enough to fuction. This is beginning to take longer. I also have been diagnosed (4years ago) with a heart blockage (30% in 1 and 40% in a second) I am presently taking a medication of metoprol (.25 mg twice daily and 1- 81mg asparin) Your advice would be greatly appreciated by an indigent person.

My son is 3 and 1/2 yrs old and he has, what I think is unusually red and puffy hands. Particularly his palm area. Some days its worse than others and it almost looks painful..I was just wondering if this could be a symptom of a problem.

Cassie, if there is puffiness in face or hands, we need to rule out disorders of kidney. For which urine test- routine and microscopic may be done. If kidneys are normal, then functioning of endocrine glands will be checked. Please refer to a pediatrician.

TB/ Liver cure related

My Father-in-law, age 58 is having serious liver problems, as well as tests show TB. He had been getting fits also from several years. He used to take alcohol, but left since few months. I urgently need to know the best doctor, if possible in Delhi/ Medical/ AIIMS. And pls tell if this is curable. Pls tell me the contact number of doctors asap.

Reena, alocoholic cirrosis is not curable but TB is curable. Please contact Gastroentrology deptt of AIIMS for liver porblem. For TB, refer to deptt of medicine at AIIMS or a Chest &TB specialist at LLR Hospital for TB at Mehrauli, N Delhi.

My mother suffering Mental strokes

Dear Sir/Madam,
plz guide me.

My younger brother was a kidney patient and he died in Dec2008. Later on my mother never became normal because of this tragedy. Now she is suffering something like mental strokes in which the whole body gets tight and she looses conciuosness. Earlear it was a matter of weekly repetation. We have gone for treatment as we were able. Now stokes are a daily routine and she is becoming very weak.
It is very tough situation for me, plz guide us.
Should we go to AIIMS and to which department.

Please provide your valuable advice for the case described below.

A brief case history:

Name: Gita Dei Age: 54, Occupation: School Teacher, Residence: Puri
My relationship: She is my Mother
Background: My father got affected with Herpes Simplex Virus (April 2nd week 2008) that affected his brain and was treated under Dr. Maya Gantayet. It was a complete stress period for my mother for four months during which it seems she did neglect her health (she has BP for which she used to take medicine. During May - Aug 2008, she was very irregular in taking medicine). She does not have any other diseases and extremely hard working and fit lady.

Phase 01: On 5th Sept 2008, while starting for school, she had reeling head and vomiting. Got admitted to Puri hospital. Since the condition did not improve with routine medicine, we took her to Kalinga Hospital Bhubaneswar on 6th Sept evening. There she started having disorientation, was treated conservatively and was continuously improving. CT scan on Sept 11th showed resolving IVH. Sodium/Potassium value was imbalanced, which was tracked to control. On 18th Sept, when she was in the verge of getting discharged, she suddenly picked-up high BP (220/100) and become unconscious. A repeat CT scan showed re-bleed in left internal capsule region with increased intra-ventricular bleed and hydrocephalous. From thereon, she went through four external drainage, each with an average length of 6-7 days and a break of 4-5 days in between. The fluid quantity was around 200ml per day (when started) to around 100ml per day (around Oct 24th). The fluid was also more transparent by then. She was treated under Dr. Chandramouli Mitra at Kalinga Hospital. On Oct 24th, the decision was to go for internal VP shunt. The VP shunt operation was done on 26th October. After this operation, she was improved slowly in neurological status. Hyponatraemia also became normal. She took feeds orally. She was discharged from medical on on Nov 26th. Her mental disorientation continued (though there was slow improvement over a period of time.

Phase 02: On Dec 14th, My Uncle brought her to Mumbai. Under Dr. Keki Turel (Mumbai Hospital), we did a thorough testing/scanning/MRI. We realized the left ventricle has moistures and the passage between left and right ventricle was blocked. We went with another operation (with all latest technology) and created a passage between left and right ventricle. After the operation, she has improved. It is four months from second operation and eight months when she got ill first.

The current status:
On Physical Aspects:
• Parkinson Tendency is developed (She is taking Valparine 500 three tabs daily)
• She was suffering from malaria for last 2 weeks (Now she has recovered from Malaria)
• Walking is not stable (After 2 weeks on 6th July she walked with out any support)
• Have better balance while getting-up/sitting-down
• Control on bladder has been decreased (Because of malaria she was very weak and down)
• Speaking is reduced after malaria showing some tendency to speak more from 5th July (both Oriya and English)
• Co-operation to do regular exercise is More than OK but not very pro-active.

On Mental Aspects: Net-Net, there is progress
• Counting (both in English and Oriya) is fairly accurate – Stumbles after 50/60 and counts correctly there after with some prompting
• Writing in English and Oriya has significantly decreased – even not willing to write.
• Reading is fully absurd – Read completely different stuffs than what is there in the content (both Oriya and English)
• Past memory recall – we observed slow improvement over last three months (like remembering poem lines once supported with lead lines, manage to identify many key days with hints– e.g., Teacher’s Day and its association with Dr. Radhakrishnan)
• She can recite many prayers both Sanskrit and Oriya now at a stretch but sounds emotional.

On Social Aspects:
• Does not recognize family members consistently. Sometimes you feel that she recognize but in the next moment she will forget (The success rate has certainly improved in last few months)
• Recognize deities/places very little
• Prefer to spend time by sitting silently
• Started watching TV for last 3-4 days and does not complain if someone watching TV/listening Radio
• Sometimes she feels she has given some of her clothes somewhere and she has to peak it up.
• Societal consciousness is excellent. Very attentive to serve the relatives with water and food. Even she accompanies every visitor to the door step to bid them good bye.
• Likes to speak over phone

Status as on 24th April:
On Physical Aspects:
• Walking is more stable
• Have better balance while getting-up/sitting-down
• Have more endurance (can walk 2KM within one hour during morning/evening time)
• Control on bladder has improved (but still has slight sensation inadequacy)
• Speaking is quite fluent (both Oriya and English)
• Co-operation to do regular exercise is OK but not very pro-active
On Mental Aspects: Net-Net, there is progress
• Counting (both in English and Oriya) is fairly accurate – Stumbles after 50/60 and counts correctly there after with some prompting
• Writing in English and Oriya has many deficiencies (hand –writing is good, control on lines are weak, spelling mistakes – mainly around vowels) – spells better while speaking rather than writing
• Reading is fully absurd – Read completely different stuffs than what is there in the content (both Oriya and English)
• Past memory recall – we observed slow improvement over last three months (like remembering poem lines once supported with lead lines, manage to identify many key days with hints– e.g., Teacher’s Day and its association with Dr. Radhakrishnan)

Lloyd, your son needs x-ray low back AP and lateral views and if possible CT scan. In the mean time an analgesic with complete bed rest may be taken. Please consult an orthopedic surgeon.

As history, he had a mild stroke three years ago with no visible consequences or affections. Is it possible that this event is a sequel of his stroke. I had period at 2th November,I did sex at 8th November without any protection,I took i-pill next day within 30 hours and did sex at 13th November with protection(condom) at 16th November my period has started again,so what could I do now ,do I need to consult with doctor?Why this happen to me?
